Community Services

District Nurses – The District Nurse team is based in the community. If you require a visit from the Community Team please contact the Care Coordination Team who will happily arrange this for you.

Healthy Family Team -The Healthy Familys team can provide advise with anything child related. They can be contacted directly on 0115 8834661 for Gedling and 01636 594809 for Newark and Sherwood.

Midwives – A team of Midwives are based at Park House Medical Centre. If you require the services of a Midwife please contact them directly on 0115 961 7616.

Referrals

Electronic Referral System

Research has shown that patients want to be more involved in making decisions and choosing their healthcare. The NHS is changing to give you more choice and flexibility in how you are treated. With this in mind, the NHS has introduced the Electronic Referral system. This system is only used when your GP has referred you onto a specialist.

If your GP decides you need to see a specialist for more treatment you will be referred to a consultant at the hospital. If the speciality you are being referred to is available under the Electronic Referral system, the following will happen:

You will receive either at the surgery or in the post, a reference number and password.
You will be given a telephone number to ring and arrange your appointment.
Where possible you will be offered a choices of hospitals, you can then make a decision on which hospital you would like to attend.

NHS e-Referral Tracking

If your doctor has referred you to a specialist for further care you will be asked to book your appointment using the ‘NHS e-Referral’ system.

You will receive a letter from us within 2 weeks giving details of how to book your appointment and any passwords you may need.

To track your e-Referral you will need the following details to log into the e-Referral website:

Your date of birth
Reference number – this will be on the first page of your letter
Password – this will be on the second page of your letter

Private Referrals

You may wish to see a specialist consultant as a private patient. In this situation, the patient is responsible for making the appointment with the specialist at a private hospital themselves. Your Doctor may be able to advise you on which consultants hold private clinics.

You may need to request for your GP to write a referral letter for you to take with you to your appointment. The private clinic will advise if this is required. There may be a charge for such letters and you must allow adequate time for us to process your request and produce the letter. You will be required to collect the letter from the surgery and make the required payment if appropriate.

Travel Health

Do you need vaccinations?

Unfortunately our Nurses are not currently providing travel health advice or immunisations. Please arrange an appointment for a travel consultation at least 6-8 weeks before departure if possible at a local Travel Health Centre such as Boots at Victoria Centre. Vaccines need time to take effect and some may require a course over several weeks.

Well Woman and Well Man Checks

NHS Healthchecks are available for this aged between 40-74 who do not currently have any medical conditions such as asthma or diabetes. This appointment requires a 20 minute appointment with a Health Care Assistant and then a follow up telephone call with a Practice Nurse. Please contact the team to book. 

New Patient Check

All patients registering with the practice are invited to attend for a new patient check. This is part of our health promotion work and provides the doctors with a basic knowledge of your health history and medications. Please bring a urine specimen to this appointment. Your paper notes can take up to 6 weeks to arrive.

Minor Surgery

After discussion with a doctor and by appointment, minor operations can be performed at the Practice.

Family Planning

Our practice nurses offer a family planning service. Please book an appointment via our care coordination team. Patients can also attend the Park House Health and Social Care Centre for the general Family Planning Service.

District Nurses and Community Matrons

Occasionally you may be referred to the district nurse or community matron service by your doctor or the hospital.

The district nurse team and community matrons are based at Park House Health and Social Care Centre and can be contacted on 0300 0830100.
